Fire Hydrant Concerns at 4 Silver Ridge Ri NW

Hydrant very low, can't make a full turn in the hydrant wrench. I know these are difficult to fix, but I will always report the low ones as this can hinder our job when there is a fire. The top nut is leaking and the plastic electrical tube is in a bad spot. The right side 65 mm port threads are wrecked as well. There are many issues with this hydrant that need to be addressed.
